LCH aims to reduce health disparities and improve life expectancy among underserved communities. We started this journey by engaging with African American men and women because this population is overrepresented in almost every chronic disease category, such as hypertension, diabetes, and various cancers. Prevention is better than a cure; to teach that message, we need to engage, influence, and reach communities in places of trust, with people they trust, and where they frequent the most.
Social determinants of health cover basic needs from transportation to food security. It’s difficult to see your Primary Care Physician annually without a way of getting there. It’s burdensome to think about your blood pressure when you’re focused on providing food for your family. It’s nearly impossible to stay on top of your monthly prescriptions when the closest pharmacy is miles away. When there are barriers to access healthcare, individual and community health suffers.

After a person completes our online Health Risk Assessment, our algorithm will recommend resources and actions for you to take. LCH sets up monthly events to address SDOH needs from transportation to food security. You might even see us in your community offering free haircuts and discussing your health needs!
If your health plan is contracted with us, we will offer a targeted and guided health journey to ensure the gaps indicated are closed. You will also be able to connect with one of our Community Health Workers via chat or phone if you need further assistance.
